- Present to any of the listed DHM collection centres to have the tests done, taking note of opening times. There is no need to book, pay or fast for any of the tests on this form. When you arrive at the DHM collection centre of your choice, they can provide you with a specific email address where you can forward the pathology form, meaning you will not have to print it.
