The final section of this registration process relates to the Monash IVF Day Surgery. It focusses on aspects of your past medical history and general health. This will best inform your caring team and help them look after you in a way that is most safe and appropriate. Unfortunately, autopopulation is not a feature of this program meaning you will need to enter your name and date of birth again – we are sorry about that!
